What do I bring with me for my first appointment?
  • If you were a patient of this office prior to 1995 (including patients of Dr. Ward Stevens), please call our office before your appointment so we can obtain those medical records from our archives.
  • Prior to your appointment, you should contact your primary care physician to obtain a referral or preauthorization if required by your health insurance plan.  Bring that referral with you.  If the referral is not in place at the time of visit, the visit may have to be rescheduled.
  • Obtain a copy of pertinent medical records from your referring physician.  This may include the last few office visit notes, blood work results, and any diagnostic studies such as X-ray reports, MRI/CT reports, EMG/Nerve Conduction Velocity reports, and Doppler or vascular studies.
  • Bring any actual X-rays, MRI/CT's etc.  A CD-ROM disc does not suffice.  We will need the actual printed films.
  • Bring your insurance card for us to photocopy.
  • Bring the completed registration packet provided by our office.  If you did not receive one via mail, you can obtain it here.

What can I expect for my first appointment?

  • When you arrive, sign in at the window. After we have entered all your information and assembled your new chart, you will be taken back to your exam room by a member of our staff.  At times, there is delay.  Please be patient.
  • A member of our staff will briefly speak to you before you see your doctor.  This will include a blood pressure check.  Therefore, please wear either loose or non-bulky clothing if you are not planning on wearing shorter sleeves.
  • The physician will examine you and will show you your X-rays/films.  He will discuss options of treatment with you.  If you have any questions, ask them at this time.
  • If you need any further services or a return appointment, those will usually be scheduled before you leave by the physician's secretary.
