What do I bring with me for follow-up appointments?
  • If you have new insurance or billing information, please bring that with you.  We will need to photocopy any new insurance cards you have.  If your previous referral has expired or you have used the total number of authorized visits, prior to any follow up appointment, you should contact your primary care physician to obtain a new referral or preauthorization if required by your health insurance plan.  Bring that referral with you.  If the referral is not presented at the time of visit, the visit may have to be rescheduled.
  • Bring any actual X-rays, MRI/CT's etc. that you have had since your previous visit.  A CD-ROM disc does not suffice.  We will need the actual printed films.  Bring the printed radiology report as well.
  • If you are coming in after you have seen a physical therapist, chiropractor or other physician that we have referred you to, please bring that physician's progress notes to your next appointment.
  • Bring an updated medication list each time you visit our office.  Remember, medications include such things as vitamins, herbals and over-the-counter products as well.
  • If you have developed any new medical allergies that you have not previously informed us of, please tell our staff. 
  • Finally, if you have a free hand and a warm heart, we always accept chocolate!
