In addition to the policies contained under the various
subsections of our "Patient Resources" page, the following policies are also in
place: |
- "No-Show" for an appointment In the event you cannot
make it to your appointment for any reason, please give us a call so that we
can offer your appointment time to those individuals on our cancellation list.
If you are a "No-Show" for an appointment and do not call to inform us of your
need to cancel or reschedule your appointment, you will be placed on a No
Further Appointment list at our office and will not be seen by this
practice.
- Medical Records We have 15 days
from the date of receipt to comply with any request for medical records.
Therefore, if you need your records, please plan ahead so that you may receive
them in a timely fashion. There is a fee for medical records,
which is a $10.00 research fee PLUS .50 cents per page. In addition,
please note that we must have a signed authorization from the patient to
release records to anyone. This includes the patient themselves,
spouses, parents, children, attorneys, insurance companies, etc. If you
need to sign a release, you can either stop by the office to do that, have us
mail you one or download one from the Forms section of
this site.
- Disability Forms We have 15 days
from the date of receipt to comply with any request to have this office
fill out your disability forms. Therefore, if you need this service,
please plan ahead so that you may receive them in a timely fashion.
There is a fee for having us fill out the disability forms, which is
$15.00 per form with the exception of the Family Medical Leave Act forms,
which we complete free of charge. Please note, we reserve the right
not to complete the forms. It may become necessary for your primary
care physician or other treating physician to complete the forms for you.
- Reasons for dismissal If you become abusive to our
staff in any way, if you are not in compliance with recommended treatment
(including, for example, not attending physical therapy or abuse of your
prescribed medications), or if you ignore our request for payment which leads
to collections efforts, you may be dismissed from this practice as a patient.
